Most leaders aren't intentionally ignoring what's happening around them.
They see what gets reported. They hear what reaches them. They work with the information available.
The challenge is that every organization has things that never quite make it into the room.

Patterns that are visible to employees but not always visible to leadership.
One pattern we've noticed in organizations is that many leadership challenges aren't caused by a lack of effort or capability. They're often the result of leaders making decisions through a very narrow window into a much larger reality.
The narrower the view, the easier it becomes to mistake symptoms for causes.
